To help make the concept of a community care hub (CCH) easier to understand for a wide-variety of audiences, including aging, disability, and community support providers, as well as their health care partners, policy makers and others, the Center of Excellence to Align Health and Social Care (COE) has developed the following resources:
- An updated definition of a CCH, and
- In collaboration with the National Association of Chronic Disease Directors (NACDD), a one-page CCH infographic combining visually appealing elements with text to present what a CCH is, what it does, who it works with – and how they work together.
