How to Join or Update Your Information on the National Map of Community Care Hubs

Discover how your organization can be included on the map or how to update or change your organization’s existing listing

The Center of Excellence to Align Health and Social Care (COE), part of the Aging and Disability Business Institute at USAging, has published a National Map of Community Care Hubs (CCHs). The map includes dozens of CCHs around the country and can be used by a variety of stakeholders, including policymakers, health systems and other health care organizations and payors, or community-based organizations (CBOs). The map will enable stakeholders to quickly and easily find and connect with CCHs, allowing them to explore details about each CCH, such as which counties they serve and products and services they offer.
