This two-minute animated video is designed to increase awareness and understanding of community care hubs (CCHs) and the important role they play in supporting person-centered health by aligning health and community care.
This CCH introductory video – the first of three – was created to make the complex concept of a CCH easier to grasp and remember for a wide range of stakeholders. The subsequent videos will focus more specifically on the value CCHs can bring to healthcare providers, health systems, and health plans.
