Health at Home Challenge from the Administration for Community Living

The Administration for Community Living ‘s (ACL) Health at Home Challenge is a competition that aims to scale advanced community care networks led by community care hubs (CCHs) that integrate health and community care to improve health, independence and affordability. It includes three phases, each offering up to $2 million in funding  spread across the awardees.
