The online, interactive map was developed by the Center of Excellence to Align Health and Social Care (COE) and can be used by a variety of stakeholders to quickly and easily find and connect with community care hubs (CCHs) across the country. Individuals can explore details about each CCH on the map, such as which counties they serve and what types of services they offer. Information is also provided for organizations who would like to be included on the map, as well as those who are on the map and wish to update their information.
