“Voices From the Field: Insights on Coordinating Health and Community Care in Rural, Frontier and Tribal Communities,” a report from USAging’s Aging and Disability Business Institute and the Center of Excellence to Align Health and Social Care features national trends in contracting for Area Agencies on Aging (AAA), community-based organizations (CBOs), community care hubs (CCHs), Tribal organizations and health partners that serve rural communities. This report highlights the results of a series of key informant interviews with local leaders and additional research into the coordination of health and community care in rural, frontier and Tribal communities.
